02. Can I order by fax or email?
Our secure online process is the preferred and most expedient method of ordering from 4MD Medical. However, we understand that there may be times when you would prefer to order by fax or mail. To order by fax or post, click Here to download a print out of our Order Form. When you have completed the order form, mail or fax it to:

We will begin processing the order as soon as it is received. Please include an email address to receive an order confirmation and tracking information on your shipment.
03. What forms of payment does 4MD Medical accept?
We accept the following credit cards: MasterCard, Visa, American Express, and Discover. We also accept Amazon Pay, Paypal, Affirm and Approve.
04. What type of Purchase Orders do accept?
Public Sector: (Federal, States and Local Governments & Education Institutions)
Medical Facilities: (Nursing Homes, Hospitals, Assisted Living and Other medical facilities)

05. Do you sell other products not listed on 4mdmedical.com?
Yes we do, and our customer service team can help. We’ll track down the item for you; and give over the information. There’s no obligation to purchase.
06. Do you accept Medicare/Medicaid/Insurance?
We don’t directly bill any 3rd party insurance payer (Medicare/Medicaid).
07. Do you ship to PO Boxes?
Due to the shipping carriers we use, we are only able to ship to physical addresses.
08. Do you ship internationally?
International customers are responsible for paying any taxes, duties, or customs fees charged by their local government. If you have any questions, email us at customerservice@4mdmedical.com. Please include your name, and where applicable, your order number.
09. Does 4MD Medical charge sales tax?
4MD Medical is required to charge sales tax on merchandise and shipping charges in New Jersey. The amount of sales tax charged is based on current local tax rates. Sales tax will be refunded for returned items. State and local sales tax rates are subject to change at any time.
10. How Do I return an Item?
If you receive a product and it doesn’t fit, or it is not what you expected it to be, most products can be returned. Products must be in their original packaging with all instruction/warranty paperwork. Product must be unused. A 25% restocking fee will apply. To return a product please request a RMA (Return Merchandise Authorization) by calling 732-905-5400 or fill out an online Return Authorization Request. You must request an RMA within 10 days of receiving the product. All returns must be completed within 30 days. Orders returned without an RMA number will not be recognized as a return and will be charged a 30% restocking fee in addition to the shipping charges, once processed.
